Patient Forms and Policies
Your first visit at Blackburn Woolfolk Dermatology involves a few steps to help make your visit smooth and efficient.
Patient Registration
We encourage all new patients to register for our patient portal.
Once you have scheduled your appointment, you will receive an email link from our Patient Portal, welcome@practicemailer.com that will provide your username and access to the patient portal.
Once you have entered the patient portal, please take time to complete your medical health history prior to your visit.

Policies
Late cancellation and no-show policy
We understand there are times when you must miss an appointment due to emergencies or other obligations. However, due to our high patient demand and limited availability of appointments, we have a cancellation fee.
If an appointment is not cancelled in advance 24 hours (regular appointments) and 48 hours (cosmetic and surgical appointments), you will be charged a fifty dollar ($50) fee that is NOT covered by insurance.
Insurances
Here are the requirements for using insurance:
- Insurance card must be presented at each visit. We accept temporary cards (available for you to download on most insurance’s website).
- Government issued picture identification must be presented at each visit (e.g. driver’s license or passport).
